Lauren Paquette Nurse's Note Healthy Reminders COVID

  • Yes, Covid is still an issue.

  • We are requiring students who test positive to remain home for 5 calendar days.

  • Please be vigilant for any symptoms your student may be having, and let one of us know ASAP if your student tests positive. (We have some health department requirements for reporting).

Prescription Medications

  • Any prescription medications will NOT be allowed without the proper authorization from a doctor.

  • The forms are always available on our website, and we can send one home with your student and/or email to you.

  • Some doctors will use their own form, this is fine; however we must have the first page of our medication form completed and signed by the parent.

OVER THE COUNTER (OTC) MEDICATIONS

  • HPA does allow OTC medications with the following restrictions:

  • The form must be completed and signed by a parent

  • The medication must be a new, unopened container of the medication.

  • (The student CANNOT carry this medication with them, it must be kept in the nurse's office.

  • The medications are outlined on the OTC form. (available on the website, can be sent home or emailed to you)

One last reminder, students MUST be seen by a nurse in order to have a nurse dismissal. If the nurse has not determined that they are sick enough to go home, it will be an early dismissal. Thanks so much. Carey & Melissa Fine Arts
